if ins. covers a br. lift, how much extra is the augmentation? silicon vs. saline

I live in St. Louis and was wondering if anyone else has ins pay for the luft then paid out of pocket for the augmentaion. If so what kind of implants did you get and how much extra did it cost> Thanks
