How can I get a pannelectomy approved by Kaiser insurance?

I have an upcoming appointment for the plastic surgeon through Kaiser to see if I qualify to have the pannelectomy procedure done. I need to know what kinds of things do they look for fo rthe approval or is there anything I can do before I get there to help? I need to know what Kaiser's guidelines for the pannelectomy are. Has anyone had this procedure paid for by Kaiser?
