Any samples of letters for an appeal to insurance if you are turned down for a TT?

I have been turned down by insurance (Blue Cross/Blue Shield)for abdominoplasty after losing over #110. I would like to write an appeal and would greatly appreciate any suggestions and samples of letters others have written to have their TT covered. I would be happy to post my letter once written to help others with this process as well. Thank you in advance!
