I need help with the appeal process!! Please can someone

give me some advice? I have Cigna HMO, and they say that I need to provide them with 2 years of my diet history, plus with that have 2 26 week supervised diets. I cannot wait another year to have this surgery, nor can I afford to pay for these supervised diets. Can anyone help me? Please?
