Any suggestions for self pay under $15,000

My insurance won't cover gastric bypass. I will do self pay. I have been exploring going outside the US for surgery but want to see if I can find a reasonably priced, yet reliable self pay in the US. Can anyone suggest a good surgeon and self pay program that is less than $15,000. A bonus would be a surgeon who is sensitive to working with patients who travel long distances for the surgery (for example, limits the number of appointments necessary at his office).
