Anybody dealt with BCBS of Texas RISK POOL?

BCBS of Texas has a written exclusion policy for the surgery. I'm not sure about the risk pool situation. Is that under BCBS or is that a totally seperate company? Does anybody have a sample appeal letter that I could see so I can write one for myself?
