Has anyone used the Kaiser Woodland Hills, CA

I'm considering having the surgery and have already talked to my PCP about it, she said she would do a referral for me, no problem. I was wondering if anyone in the group had gone thru the Woodland Hills, CA facility for Kaiser. If yes, what did they require and where were the appointments (at Kaiser Woodland Hills) or at UCLA main center or UCLA Santa Monica. I've posted on the Kaiser site, but so far no responses.
