Anyone been approved from Humana ChoiceCare withOUT Dr-supervised diet?

I have Humana ChoiceCare and wondered if they will approve without physician-supervised diet? If so, what were your circumstances? I have tried a nutricianist supervised wieght loss, but it was when I was 11 and the hospital no longer has the records for this. Thank you!
