Are there any independent insurance policies that don't exclude WLS?

I have been researching the possibilities of self-pay vs. independent insurance. I know that it would be wiser to have insurance as backup in case there are complications, but I can't seem to find a policy that doesn't exclude either overweight people or have an exclusion when it comes to WLS. I have even researched the ones that charge in excess of $200 a month for a policy but to no avail. I am currently located in Indiana, I am a resident of California, but I am relocating to Colorado in the next few months so I could make any of the three states work if someone knows of a policy that would work for me.
