Alabama pre and post ops I need help!

Can anyone help me please. I have already been for a surgical consultation with Dr. Andrew Dewitt in Bessemer Alabama only to find out that my insurance has an obesity exclusion in it.. On top of that, my husband lost his job. So, if I keep this insurance with the obesity exclusion in it, the cost will $502.00 a month is it worth it to keep this insurance and try for an appeal? I have no PCP really that has seen me over the years and I also do not really have any Dr. supervised dieting. Will I be wasting money and time? My other question is. I am legally not married so considered a single mother of 3. Can I get on medicaid and get them to pay for this surgery or would Alabama Vocational Rehab pay for it?
