Any of Dr. Wongsa's patients do their surgery self-pay?

I am just wondering what the price range is to be prepared for if my insurance still refuses to deny my appeal. Also, in case my insurance does approve, how much were the total costs if insurance picks it up? (I still have to pay 20%).
