"lifetime" benefits?

I know I meet all of the insurance criteria for a revision, (and there is specific criteria for revisions) but I am not sure  on one point.  My insurance, Preferrred One has a note on the policy that they will cover one weigh loss surgery per lifetime per patient.  The only way their 2 criteria can work in my opinion is that they will cover the revision if a different insurance covered the primary WLS....what do you all think. thanks, and I hope this makes sense.

You can certainly call the number on the back of the card  and CS can help clarify things like this because each company and policy is different.... but i work in the health insurance field and generally when they say things like per member per lifetime it means for that policy THEY will cover one weight loss surgery per member per lifetime if you had the first under a different insurance company or even though they dont want you to know this usually even if it was with the same company but a different policy like if you changed employers the claims will probably proccess as the first wls per member per lifetime if that makes any sense.... hope that helped
