CIGNO HMO GA and Plastic Surgery Reimbursement

I was always under the impression that PS was not paid for thru insurance (at least not without a good fight).  That said, I did 3 procedures (TT/ Arms / BA/BL) in 12/06 and I am scheduled for inner thighs on 9/4/07. 

I was scanning the post to see what to expect after surgery from the inner thighs and found some post stating that CIGNA was difficult in approving WLS but the approval process for plastics was a breeze.  I have CIGNA HMO. 

I wanted to solicit feedback from the experienced.  See below

Question

Is it possible to receive a reimbursement on services already rendered thru your insurance company, if a medical case is proven?

If yes, how do I start the approval process?

I was wondering if I could file a claim with CIGNA for the services I paid for in 12/06.  If they sent me a fraction of what I paid, I would be ecstatic.
