PLEASE, Member Services, if you're going to re-post a survey or something like this post it and CHECK IT FIRST by filling it out and submitting it yourself as the first respondent.
That way you'll actually know what its like from our end. TY !! 
I am sorry it is not working for you. We have tested it and it is working properly, including the uploads and transmission of the submission to the company. Our IT Dept seems to think it is your connection. You might try going to a library (if one is open where you are) and attempting to do it from there. Hotspots could work, but some get bogged down depending on location and number of people on it. If all else fails, you can email your information and photos to [email protected] and we will forward them over to the contact.
