question about Packet
I recieved my packet in the mail to complete. My PCP called Dr. Fuzz's office and it was in the mail. My question is do I sign release forms and have all medical info forwarded to PCP office? There are so many question to answer in the packet. For instance, going back to 2003-2008 my weight each year. Well I guess all of medical records from Dr. over the past 5 years would have that. All I am sure of that I have gained weight every year. My BMI is 38.9 and I have high cholesterol and high blood pressure. My knees and hips hurt as well. I have attended on WLS support meeting and I was told that Dr. Fuzz is having one Nov. 20 as well as Nov. 25 at MedHealth at Hanes Mall. Once I complete the packet and send back will I hear from his office at all? I have BCBS PPO insurance. I have so many questions and I know this process takes a long time. If you have any advice, please let me hear from you.
You will fill out all the questions and send it. Please make a copy before sending in. Sometimes the Bariatric surgeon office will loose. I know several people that have had their records lost.
Normally your primary care physician just wants you to send the information on directly. I did get a copy of all of my medical records for the last 5 years so that I could write every thing down that I had tried. Luckily my physician had written patient on atkins, patient doing weigh****chers. This helped me have a accurate record of what I had tried. The insurance companies want to know that you have tried to loose weight with little or no success.
Is your Bariatric surgeon office a Center of Surgery of Excellence.
If you have questions ask on the board. Everyone is great. Read everything you can on the surgery- books and on the board.
