Your doctor can call and get the medical critera for BCBS of Michigan.  As long as you can substantiate that you have met the medical critera then you will be ok with the surgery.  BCBSM does not do pre-auths for Gastric Bypass surgery unless you do not meet the medical critera - so a letter from them is out of the question.  Have your surgeon give them a call and they can explain it all to them.

It's your surgeon's responsibility to make sure you meet all the requirements of BCBS before they perform the surgery ---- or they won't get paid once they bill it.  So get copies of all the paperwork that proves you met all the requirements (medical history/chart, dieting history, psych eval, pcp letter, etc.).  That's all you need to do. Good luck! Pam
