Call your PCP's office and talk to the office manager if you can. There should be lab reports and other documentation in your chart. If not, that's terrible recordkeeping on the part of your doctor. Every time you visit, they should be taking your weight, BP, medication history, etc. Emphasize that you need records that show documentation of these co-morbidities and any attempts at dieting. If you had surgery, you can request the records from the hospital where you had the procedure. I would also request a letter from your PCP documenting your co-morbidities and a letter from the surgeon who did your knee surgery with his/her medical opinion that your weight contributed to the need for surgery.

Martha, I am not sure if this really sent my insurance company over the edge but my PCP put in my mother and father's illnesses such as my father recently passed from heart and diabetes problems. Hope that helps!
