If you go to Atena's web page they list their requirments under bulletin 157. First you have to check to make sure your company has the WLS rider with the insurance and if they do then you have to do exactly what Aetna requires. If you do it to a tee then you should get approved. Make sure your doctors document every visit. They want a lot of documentation. Best of luck. Take care.
