United HealthCare Requirements?
Can anyone please tell me what the Requirments are to have Gastric Bypass Surgery threw United Healthcare? Everything that you know that they require will be a great help......and any Dr.s in the area that except United, and if anyone knows if Dr. Murr in Tampa excepts United Healthcare........everything will be appreciated...hugs, Maria
I know that Dr Rehnke in St Pete accepts UHC. He is doing my lap band next Tuesday. His office is very familiar with them as they got me approved in 4 days. He performs both lap band and RNY.
He has a website.....obesitynomorefl.com and he comes to Sarasota to do seminars.
I had a few documented comorbidities (ie. diabetes, hypertension, arthritic knees)

Drs Gordon and Gonzalvo of West Coast Surgical Specialists accept United Healthcare, they do RNYGB and are starting to do Lap Band in January. They are excellent. Both did a 1 year fellowship down at the Cleveland Clinic in Westin Florida on Laparoscopic Gastric Bypass. I would recommend them highly. I would suggest you check with United about their requirements. If it is a covered service under the policy you have they can tell you what they require for approval. Most insurance companies go by the NIH standards which means a BMI greater than 40 or if between 35-40 must have co-morbidities. Such as diabetes, sleep apnea, high cholesterol, cardiac or respiratory problems, GERD, high blood pressure, back, knee pain etc. Most insurance companies also require anywhere from 6months to 1 year of a medically supervised attempt at weight loss. What that means is that for the designated period of time you have gone into your doctor's office monthly, been weighed and discussed the diet plan you are attempting to follow and that this is documented in the physician's progress notes. Dr Murr does take UHC I just had my WLS with him in Oct. and UCH covered it.Susan at Dr. Murr's office is very helpful.She helps make sure all I' are dotted and T's are crossed. Besure you get a referral from you PCP. Unfortunatly the more problems you have the better, so make sure the insurance knows about everything. Diabetes, high bloodpressure,thyroid,depression,etc....

The first thing is you need to check is your policy. The requirements are there. I had my RNY by Dr. William Todd Overcash in Ocala, who accepts United Healthcare and doesn't have any upfront program fees that some charge (352) 368-2828. I know that there have been changes in some if not all insurances so you have to check cause now for us UHC is not allowing the surgery whether HMO OR PPO. So I thank God that I did it when I did cause now it is not covered...that could be due to my job. Good luck to you! If you need anything let me know!
